The entrance is not always up if someone has cleared it recently. You must wait several minutes for it to respawn at one of the given locations.
You may reenter this instance as many times as you like, until the door vanishes and you must find it again. The outside of the door will always be guarded by a tainted scorpion
There is a heroic version also.
