What does this information mean?

This is the basic helm recipe. The crafted helm can be upgraded to a mythical version. You need to gather four components. Note: can only be performed once as the materials come from achievements and are not dropped by mobs.

1. Defeating Mastersmith Naghig Dakgu in Furnace of Ages (Challenge Duo) give the achievement Stoking the Forge which yield a Soaked Coal.
2. Defeating Tuatog the Tidal Lord in Abyssal Aqueduct (Challenge Duo) gives you a Purified Cave Water
3. Defeating K'zan the Automaton in Antechamber of the Automaton (Challenge Duo) give the achievement Automation at it's Best which yield a Mote of Gleaming Aqua Metal.
4. Defeating Gen'Ra in Challenge Mode to get the achievement Gen'ra and the Warcouncil's Demise (Challenge) which yield a Vial of Ancient Tavalan Blood.

The helm can only be crafted (commissioned) by an armorer, and can only be created once per character as the components come from achievements, not chests.

After the helm is created, there are two more steps to create the mythical version.

1. Defeat Overlord Talan in Challenge Mode to get the achievement Overlord Talan's Demise (Challenge) which yield Talan's Prized Hydra Scale.
2. Defeat Psyllon'Ris in Challenge Mode to get the achievement Psyllon'Ris' Demise (Challenge) which yield Pair of Ancient Gold Flecked Rubies.

With the last two ingredients in you backpacks, examine the helm twice to upgrade it.

The first upgrade yield a Hydra-Scale nnn of Psionic Shielding depending on class.

The last upgrade yield a mythical Indestructible Hydra-Scale Helm of nnn, depending on class.
